Overview
As research into therapeutic psychedelics expands and nonclinical community use rises, frontline clinicians increasingly encounter acute physiological and psychological presentations related to psychedelics (e.g., psilocybin, MDMA, LSD, DMT, ibogaine, ketamine, ayahuasca). This is the first conference of its kind to offer psychedelic education tailored to first responders (EMS, fire, law enforcement) and front line clinicians in the Emergency Department (nurses, advanced practice providers, emergency physicians, social workers, emergency psychiatrists, emergency pharmacists, poison control center pharmacists). This is an exciting one-day conference led jointly by Dr. Patil Armenian (UCSF Fresno Dept. of Emergency Medicine & Div. of Medical Toxicology) and Dr. Gul Dolen (UC Berkeley Center for the Science of Psychedelics, Dept.. of Neuroscience, Dept. of Psychology), with dynamic speakers who are leaders in the field. It features education on cultural and historical context of psychedelics, neurobiological framework, pharmacology and toxicology, recognition of typical/atypical presentations, triage and stabilization, behavioral risk assessment, de-escalation, medicolegal considerations, and harm reduction.

Objectives
As a result of the education, I intend to:

  1. Apply an evidence-based approach to recognizing and differentiating common psychedelic-associated presentations, including expected alterations in perception and behavior, acute toxicity, hyperthermia, psychiatric emergencies, and complications related to polysubstance exposure, in order to improve initial assessment and risk stratification.
  2. Assess the medical and behavioral risk of patients experiencing psychedelic-related crises and choose appropriate triage, stabilization, monitoring, and disposition strategies in prehospital and emergency department settings.
  3. Demonstrate trauma-informed, patient-centered de-escalation strategies when managing patients experiencing acute psychedelic-related behavioral or psychological distress, while minimizing unnecessary physical restraints, chemical sedation, and other potentially avoidable escalation of care.
  4. Apply evidence-based toxicologic and harm-reduction principles to the management of acute psychedelic adverse experiences, including recognition and treatment of serious physiologic complications such as hyperthermia and identification of circumstances requiring escalation of medical or psychiatric care.
  5. Integrate current evidence regarding psychedelic pharmacology, neurobiology, emerging therapeutic applications, and evolving legal and policy considerations into clinical decision-making, patient communication, referral, and coordination of care for individuals encountered in prehospital and emergency settings.
