Overview

Wilderness and international environments create special challenges to the delivery of health care, not only because people traveling in remote regions are exposed to elements such as high altitude, extremes of temperature, and toxic exposures from plants and animals that they would not experience in an urban setting, but also because these environments are such that if a person is injured or falls ill there will be a delay in reaching definitive care and any treatment they receive in the field will necessitate ingenuity and improvisation. Wilderness and travel medicine does share one common theme with medicine that is practiced in more traditional environments: Prevention is always better than attempts to cure illness or treat injury after it has occurred.

High altitude illness is probably the best studied area of wilderness medicine and provides a wealth of examples of the difficulty of providing care in extreme settings, the importance of health care providers having sufficient knowledge and skills, and the need to focus on preplanning and prevention.

Yosemite Land Acknowledgment Statement
Yosemite National Park currently sits on land belonging to the Ahwahneechee People.

Ahwahnee, known as Yosemite Valley, is now one of the country’s most famous National Parks, an icon of John Muir’s conservation movement and America’s best idea, but before this public “wilderness” was constructed, it was home to the Ahwahneechee People. The Ahwahneechee experienced multiple violent removals from the would be park beginning in 1851 when their villages were burned to the ground by the California Militia and ending in 1969 when their homes were again burned for a firefighting drill. Despite the U.S. government’s claim that the Ahwahneechee are extinct, members of the tribe are still fighting for Federal Recognition and use of Ahwahnee for cultural practices.
